Evaluation of enthesopathy in patients with type 2 diabetes mellitus with subclinical arthropathy

Abstract:

Aim of the work: To assess the presence of enthesopathy in patients with type 2 diabetes mellitus (DM) with subclinical arthropathy. 
Patients and Methods: This cross-sectional study was carried out on 50 patients, aged from 19 to 65 years old, both sexes, with Type 2 DM. Patients had been allocated into two groups: with and without enthesitis regarding to Madrid sonographic enthesis index (MASEI) scoring to establish a correlation between them regarding to sex, age, duration of DM, lines of treatment of DM and control of DM by glycated hemoglobin (HbA1c).
Results: MASEI score is considered the best early predictor of inflammatory enthesitis in cases of type 2 DM with p value = 0.009. The most affected site with enthesopathy is quadriceps tendon then planter fascia. Evaluation of triceps tendon according to MASEI makes it superior to other scores as it is less affected in other causes of degenerative enthesitis. Doppler signaling can detect active enthesitis early. 
Conclusion: MASEI score is considered the most specific and sensitive method in diagnosing enthesitis and differentiating it from other causes of the same complaint in diabetic patients.
